Requirements for Assistance (must fill out a needs form)
Prior to filling out the Service Request Form and before items can be provided, Kindred Heart Families requires the following information:
Foster Parents
• When child was placed with you
• Driver’s license
• Proof of Address
• Court ordered foster placement paperwork for each foster child
Kinship and Non-Kinship (NonRelative) Includes Grandparents raising Grandchildren Caregivers
• When child was placed with you
• Driver’s license
• Proof of Address
• Paperwork showing that child(ren) resides with you, including but not limited to court paperwork, school registration, child’s EBT/Badgercare card, etc.

Adoptive Caregivers
• When child was adopted by you
• Driver’s license
• Proof of Address
• Paperwork showing that child(ren) has been adopted
Social Workers and Guardians ad Litem
• ID badge
• Entire contact information required on our Needs Form
